摘要

目的 探讨超声辅助下椎管内麻醉在肥胖产妇中的应用效果。方法 选取2022年6月至2024年6月在江苏省常州市中医医院进行分娩的96例肥胖产妇,进行剔除脱落后,共有90例产妇进行研究,按照麻醉方式分为对照组、研究组各45例,对照组进行传统触诊定位下椎管内麻醉,研究组进行超声定位下椎管内麻醉。对比两组穿刺情况、生命体征、麻醉效果、新生儿Apgar评分、并发症。结果 研究组穿刺次数、麻醉前准备、穿刺、起效时间短于对照组(P<0.05)。麻醉后10 min,研究组舒张压、收缩压、心率低于对照组(P<0.05)。研究组术后2 h、4 h、6 h 视觉模拟量表(VAS)评分低于对照组(P<0.05)。研究组麻醉Ⅰ级患者占比较高(P<0.05)。研究组新生儿1、5、10 min Apgar评分高于对照组(P<0.05)。结论 肥胖产妇分娩时采用超声下椎管内麻醉,可缩短起效时间,减少穿刺次数,抑制术后并发症,临床使用效果较好。

Abstract

Objective Exploring the application effect of ultrasound-guided spinal anesthesia in obese parturients. Methods A total of 96 obese parturients who gave birth at Changzhou Traditional Chinese Medicine Hospital in Jiangsu Province from June 2022 to June 2024 were selected for the study. After excluding dropouts, a total of 90 parturients were selected and divided into a control group and a study group, with 45 cases in each group. The control group received traditional palpation guided spinal anesthesia, while the study group received ultrasound guided spinal anesthesia. Compare the puncture situation, vital signs, anesthesia effect, neonatal Apgar score, and complications between the two groups. Results Number of punctures, preparation before anesthesia, puncture, and onset time in the study group were shorter than (P<0.05). At 10 min after anesthesia, diastolic, systolic, and heart rate were lower than the control group (P<0.05)..The VAS score of the study group at 2 hours, 4 hours and 6 hours was lower than that of the control group (P<0.05).The proportion of anesthesia grade Ⅰ patients in the study group was relatively high (P<0.05). Neonates with 1,5 and 10 min Apgar scores were higher than the control group (P<0.05). Conclusions Ultrasound assisted spinal anesthesia is used during delivery for obese mothers, which can shorten the onset time, reduce the number of punctures, and suppress postoperative complications. It has a good clinical effect.
